Full-time Family Medicine NP or PA needed in McAllen/Edinburg area to provide comprehensive primary care services. This role is part of an integrated healthcare system with six hospitals and offers access to freestanding ERs and a specialty referral network. The position involves supporting physicians in value-based care and HEDIS measures, providing patient support via in-person consults, telemedicine visits, and sponsored health events. The role champions a comprehensive, coordinated model of care for Medicare-aged and Medicaid populations, and supports the attestation process by collaborating with physicians, managed care resources, and regional quality teams. Additionally, the role assists with ER and inpatient discharges to ensure timely follow-up appointments, covers for physicians during absences, and rotates on-call duties. The NP/PA will also educate patients on appropriate use of emergency vs. urgent care, collaborate with physicians on coding and documentation, and ensure all preventative screenings are completed.
