About The Position

The Family Medicine Care Coordinator works within a multi-disciplinary health care team in the delivery of comprehensive services for patients diagnosed with chronic illness and high risk patients in a primary care setting. Acting as the primary contact person for the family, the Family Medicine Care Coordinator assists the health care team in the development and implementation of a health care plan tailored to the needs of the client and the client's family in order to promote continuity of care, improves referral tracking, and ultimately reduces the rate and severity of diabetes-related complications.

Requirements

  • Bachelor's degree in Health Education/Health related field or California state licensure for vocational nursing (LVN).
  • Bilingual in English and Spanish required, with excellent written and oral communication skills.
  • Ability to work independently and as team member.
  • Cultural competency required.
  • Excellent organizational and problem solving skills and ability to prioritize.
  • Current CA Driver's License, available vehicle, and valid liability insurance.
  • Experience with basic computer skills including word processing and data entry.
  • Ability to perform a variety of tasks after training.

Nice To Haves

  • Experience in practical nursing, preferably in an ambulatory care facility.
  • Health education background in chronic disease.
