About The Position

In 2025, Catholic Charities opened Hunt Family Housing (HFH) on its Helen Hunt Campus, providing 24 units of transitional housing for families experiencing homelessness. We are seeking a Life Coach to work alongside families as they build stability and make progress toward safe, permanent housing. The Life Coach provides intensive, whole-family case management, partnering with families to identify goals and address interconnected needs such as housing, employment and income, education, mental and behavioral health, parenting and family well-being, and access to community resources. Using a family-driven, strengths-based approach, the Life Coach helps families navigate periods of instability and crisis while building on their strengths and supporting measurable progress toward their goals. Managing a caseload of approximately 7–10 families, the Life Coach develops meaningful relationships with families throughout their up to 18-month stay at HFH. Through assessment, case planning, advocacy, resource connection, and ongoing support, the Life Coach helps families build stability, strengthen self-sufficiency, and move toward permanent housing. This role is well suited for someone who builds strong relationships, uses sound judgment, and wants to make a meaningful difference through trauma-informed, goal-oriented family support.

Requirements

• A Bachelor’s degree is required.
• A minimum of two years of relevant experience in human services, social work, case management, or a related field is required.
• Experience must include foundational case management skills, including the ability to engage clients, identify needs, collaboratively establish goals, connect clients with resources and services, and support measurable progress toward those goals.
• Strong verbal and written communication skills, with an emphasis on listening, problem solving, and maintaining positive working relationships.
• Strong interpersonal and human relations skills with the ability to form and maintain positive relationships with diverse families and colleagues.
• Ability to apply culturally appropriate skills in interactions with clients, co-workers, volunteers, partner agencies, and the community.
• Ability to complete required on-the-job training in English within established time limits, including passing training assessments and earning necessary certifications (with accommodations, if applicable).
• Basic math and analytical skills to perform calculations (e.g., budgeting, income vs. expenses, monthly vs annually) and interpret results to support case management.
• Ability to follow directions and interpret policies and procedures to ensure compliance.
• Exceptional attention to detail, particularly in data entry, documentation, and record-keeping.
• Computer proficiency, including word processing and detailed data entry. Ability to learn and utilize various databases.
• Knowledge of Human Services agencies in the area.
• Ability and willingness to work within the established structure and guidelines of Catholic Charities.
• High level of self-motivation, enthusiasm, and team-oriented work ethic, with the ability to prioritize projects and manage multiple responsibilities effectively.
• Valid driver’s license with a satisfactory driving record; reliable transportation required for local travel between schools, home visits, and Catholic Charities offices.
• Ability to work a flexible 40-hour workweek.
• Must successfully complete background clearance.
• Must be able to regularly climb stairs, as the job requires.

Nice To Haves

• A Master’s degree in Social Work or a related behavioral-health field is preferred; pursuit of applicable licensure is a plus.
• Four or more years of directly relevant experience is preferred, particularly providing in-depth case management to individuals or families experiencing homelessness, housing instability, or crisis; supporting goal-driven progress across interconnected areas of stability; and responding effectively to complex needs.
• Bilingual in English/Spanish is strongly preferred.
• Knowledge of child development (Protective Factors) and 2Gen (from supervisors) strengths-based family support practices (FHI training), and ability to apply these practices when working with families and co-workers.
• Knowledge and application of Trauma Informed Care and Motivational Interviewing principles and practices.
• Working knowledge of behavioral-health concepts, including the ability to recognize mental-health needs, understand their potential impact on family stability and goal attainment, and appropriately engage Behavioral Health and other clinical resources.
• Proficiency in problem solving, planning, mediation, and conflict resolution.
• Demonstration of empathy and compassion, using calm, quiet, and natural tones in interactions.
• Ability to respond calmly and appropriately to families experiencing crisis, using sound judgment, maintaining professional boundaries, and seeking consultation or escalation when needed.
• Demonstration of patience, understanding, and professional courtesy in team interactions, with contributions to daily team operations including welcoming guests, maintaining shared spaces, and supporting walk-in clients.
• Ability to use a Family Development approach to strengthen families and build support systems.
• Knowledge of family dynamics, systems theory, domestic violence, and social work theory/practice.
• Understanding of social issues such as poverty, gender bias, substance abuse, child maltreatment prevention, and cultural diversity.
• Knowledge of community resources and systems.
• Commitment to ethical standards, including understanding and upholding The Code of Ethics for Client-Facing Positions through ethical decision-making, integrity, and professionalism.

Responsibilities

• Reach out to newly assigned families within 3 business days to offer case management and program services through a variety of outreach strategies (e.g., phone calls, texts, in-person visits).
• Conduct information sessions with families, when needed, to review program services, expectations and next steps to evaluate interest and change talk.
• Engage families after program enrollment and complete needs assessments using the Colorado Family Support Assessment 2.0 (CFSA 2.0) and other grant-designated tools. Provide intensive, strength-based case management services and collaborate with families to set goals using a family-led approach. Conduct regular follow-up to assess goal attainment and referrals needs. Manage a caseload of 7-10 families.
• Schedule appointments at times and locations convenient for families, creating a warm and welcoming environment. Meet contact standards based on the intensive case management model and conduct visits weekly, semi-monthly, or monthly, based on time in the program and family needs. Build rapport, assess needs, set family-centered and strengths-based service plans, provide direct services, and connect families with resources to address their concerns and priorities.
• Develop effective case plans with families regarding parenting skills, family issues, economic stressors, parent/child relationships, and community connections. Track measurable progress toward goals, acknowledge family strengths, resilience, and resources. Work toward keeping children safe in their homes and stable in school settings. Promote protective factors with families to ensure child safety.
• Work collaboratively with families to identify next steps and develop a family-centered exit plan, with the intent of transitioning from transitional housing to permanent housing at approximately 18 months. Actively communicate with and seek support from supervisor, including information on referrals received, home visits completed, and allocation of funds.
• Document agreed-upon next steps and assign responsibility for each task. Conduct timely follow ups with families to assess progress and success with referrals.
• Advocate on behalf of families with external agencies, such as welfare offices, public benefit agencies, landlords, and schools, based on individual and family needs. Support family members in advocating for themselves.
• Engage multiple family members in program activities when appropriate and facilitate opportunities for families to build social connections and healthy relationships.
• Respond to financial requests (payments, gift cards, etc.) appropriately, following established request and approval processes.
• Maintain up-to-date Release of Information (ROI) documentation annually, ensuring compliance with privacy standards.
• Consistently and accurately collect and enter data into designated database systems within five (5) days of service. Maintain organized and complete client files, program statistics, demographics, and reports, ensuring accurate and timely documentation and record-keeping.
• Seeks feedback from families and team members for program improvement.
• Participate in ongoing training and adhere to Quality Standards for Family Strengthening & Support and Trauma-Informed Care practices.
• Participate regularly in case conferencing with the Life Coaching team and Clinical Director. Attend wrap-around meetings with staff who support HFH families, required sessions with grant providers (e.g., CDEC), and other relevant professional development opportunities.
• Participate regularly in appropriate community meetings
• Travel regularly between sites – Catholic Charities, home visits, and participating partner organizations and schools.
