The Intercept® program is an evidence-based intensive, in-home program designed to keep families together by preventing out-of-home placements or facilitating reunification. It is recognized by the Title IV-E Prevention Services Clearinghouse for its effectiveness in reducing out-of-home placements and accelerating permanency for children. The program serves youth from birth to age 18 facing serious emotional or behavioral challenges or who have experienced trauma. Services are delivered directly in the family’s home and community by highly trained Family Intervention Specialists. Intercept provides intensive, strength-based support tailored to each family’s unique needs, utilizing evidence-based mental health interventions and developing new parenting and communication skills to achieve lasting change.
