About The Position

The Family Coach plays a key role in delivering community-based home visiting services to families with newborns in Allegheny County who have complex needs. As part of the Hello Baby team at Healthy Start, the Family Coach utilizes the Camden Coalition COACH model to provide high-quality case management support including screening, assessment, goal planning, and community referrals. The ideal candidate is passionate about family systems, knowledgeable about community resources, and skilled in both written and verbal communication. Prior experience in case management and home visiting is required.

Requirements

  • Case Management & Home Visiting Experience – Strong background in providing direct services, assessments, and goal planning for families.
  • Care Coordination – Ability to connect families with appropriate services and follow up to ensure continuity of care.
  • Strengths-Based Approach – Understanding of trauma-informed care, motivational interviewing, and asset-based community engagement.
  • Assessment & Problem-Solving – Skilled in evaluating family needs, identifying challenges, and developing action plans.
  • Relationship Building – Ability to establish trust with families and maintain partnerships with community agencies.
  • Flexibility – Ability to adjust to changing family needs, program requirements, and unexpected challenges.
  • Emotional Resilience – Capacity to handle sensitive situations and maintain professionalism in high-stress environments.
  • Advocacy – Commitment to supporting families in navigating complex systems and accessing needed resources.
  • Time Management – Capacity to manage multiple cases, meet deadlines, and prioritize tasks effectively.
  • Attention to Detail – Ensuring accurate documentation and compliance with protocols.
  • Proficiency in Technology – Familiarity with case management software, Microsoft Office products, and virtual meeting platforms.
  • Verbal & Written Communication – Strong skills in documenting case notes, writing reports, and effectively communicating with families and service providers.
  • At least 3-5 years of progressively responsible case management and home visiting experience.
  • Act 33/34 and FBI clearances are required.
  • Valid unrestricted Pennsylvania Driver’s License and have reliable transportation.
  • Must possess valid and current automobile insurance and be listed as an insured driver.
  • Fully vaccinated for COVID-19

Nice To Haves

  • Bachelor’s degree or Master’s degree, preferably in social work, human development, early childhood studies or a related field.
  • Significant experience/training in lieu of degree may be considered.

Responsibilities

  • Implement and promote the Camden COACH model in service delivery.
  • Apply a strengths-based, “whatever it takes” approach to assisting families with complex needs.
  • Connect families to essential services, including housing, home visiting programs, substance use treatment, mental health support and early childhood resources.
  • Establish and maintain relationships with community partners and service providers.
  • Conduct case management assessments and collaborate with families to develop individualized care plans.
  • Support families in setting and achieving care plan goals, and monitoring progress over time.
  • Accurately document service delivery in accordance with program guidelines and protocols.
  • Collaborate with the Hello Baby team, family support centers, and community resources.
  • Participate in case reviews, reflective supervision, required trainings, and team meetings.
  • Perform other duties as assigned by the supervisor.
