Metropolitan Family Services (MFS) is seeking a full-time Family Coach to join their Calumet Team.   • Requires mobility to work in the community with assigned caseload.
  • Ability to work effectively with diverse individuals and groups.
  • Works evening and weekend hours as needed and assigned.
  • Bachelor's Degree in Social Work or related field from an accredited college or university Preferred or HS Diploma plus one to five years of case management experience, which includes the coordination of youth and family services.
  • Demonstrated ability to render adequate service to clients and to organize and manage job efficiently.
  • Requires mobility to work with community-based caseload.
  • Ability to maintain assigned community and home-based services.
  • Ability to work effectively with diverse individuals and groups.
  • Must be highly organized and knowledge of Electronic Health Record, Excel and Microsoft Suite to track data.
  • Work with youth in care or history of foster parenting preferred.
  • Works effectively with diverse staff and service populations.
  • Accountable for providing exceptional customer service.
  • May need to work a couple later evenings to meet the clients' needs.
  • Satisfactorily pass all relevant screenings and criminal background checks.
  • Driving for work required with a personally owned vehicle.
  • Travel between sites required.

Responsibilities

  • Have a deep respect for a family’s expertise, which is fundamental to establishing trust.
  • Help the family to identify strengths and then work with the families to utilize these strengths and build others.
  • Use of trauma informed practices.
  • Monitors family goals and activities.
  • Provides follow up to ensure family goals are progressing or achieved.
  • Coordinates events and activities within the program to meet client needs and engage individuals in services.
  • Provides support services including transportation, child care assistance, clothing and uniform assistance as identified.
  • Records family progress in case files.
  • Handles emergency and crisis situations with families.
  • Works within contract, grant, and program requirements including meeting timelines and reporting requirements.
  • Utilizes a team approach and works with other program staff in the planning and implementation of services.
  • Takes increasing responsibility for own professional development, utilizing supervision and other developmental opportunities within and outside the agency to enhance skills and understanding.
  • Develops and maintains contacts with community groups relating to program needs.
  • Other programmatic duties as assigned.
