About The Position

The Family Centered Clinician provides community-based therapy to youth and their families. The role involves home-based, evidence-based therapy using interactive and participation-focused methods, along with diagnostic evaluations and individualized, strength-based treatment planning. The clinician connects families with local community resources, helps them navigate outside services, and prepares aftercare plans to support long-term progress. The position also includes maintaining timely clinical documentation and client privacy, attending regular team meetings, and coordinating with referral partners. Requirements

• Active LPC or LSW license, or ability to complete licensure requirements by the time of hire.
• Strong listening skills and empathy.
• Ability to build rapport quickly with families.

• Comfort working in home-based settings.
