About The Position

The CCSD Community Market Case Manager is responsible for providing comprehensive case management services to individuals and families in need of food, housing, supportive services, and community resources at CCSD's Family Support Center. This position focuses on supporting clients in achieving stability and self-sufficiency. Key responsibilities include creating individualized service plans, connecting clients to essential resources, and delivering ongoing support to ensure long-term success.

  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
  • Proficiency in Google Workspace including Gmail, Drive, Docs, Sheets, Slides, Google Meets and Calendar.

  • Bachelor's degree in Social Work, Human Services, or a related field, or an equivalent combination of education and relevant work experience
  • 3+ years of Non-Profit experience
  • 3+ years of experience in intensive case management
  • Fluent (verbal and written) in both English and Spanish
  • Experience working directly with diverse populations, including vulnerable or at-risk individuals
  • Must possess valid unrestricted driver's license and reliable transportation
  • Must obtain and maintain a valid Nevada Food Handlers Card within thirty days of hire due to required participation in food distribution events.
  • This position is contingent upon the successful completion of an intensive background check conducted by the Clark County School District (CCSD), which includes LiveScan fingerprinting. All applicants must be willing to undergo this process. Placement at school-based sites will not proceed without full clearance. The Just One Project adheres to all CCSD security and compliance protocols to ensure the safety of students, staff, and the community.

Responsibilities

  • Conduct intake assessments to identify clients' service needs and barriers hindering sustainable self-reliance.
  • Develop comprehensive individualized action plans with clients that promote self-reliance, outlining achievable goals and strategies focusing on housing, employment, health, and other essential needs.
  • Provide continuous case management services by monitoring client progress, reassessing needs, and evaluating plan effectiveness.
  • Conduct regular monthly assessments with clients to ensure they are receiving the necessary assistance and support.
  • Work closely with clients to motivate and reinforce independence, self-sufficiency, and self-esteem throughout their journey.
  • Provide immediate support and intervention during client crises; develop safety plans and connect clients to emergency services when necessary.
  • Mediate issues with clients that threaten their self-sustainability.
  • Assist clients in identifying and accessing community resources, including housing programs, mental health services, employment opportunities, and financial assistance.
  • Establish and maintain an effective and timely referral network; facilitate referrals and ensure clients access available resources.
  • Identify suitable shelter options and assist clients in securing and maintaining safe, stable housing.
  • Assist clients with budget plans and provide guidance on financial literacy, budgeting, saving, and debt management.
  • Assess client's employment skills, interests, and goals; collaborate with the workforce case manager and connect clients with job trainings and employment readiness support.
  • Aid SNAP program participants in completing applications, collecting required documents, and providing necessary data; connect clients with nutrition education support.
  • Collaborate with CCSD administrators, counselors, and family support staff. Actively network to promote The Just One Project's market services, identify opportunities for outreach, and increase engagement and participation among students and families in need.
  • Serve as a liaison to ensure smooth coordination and communication between schools and market operations.
  • Ensure the market remains well-presented by regularly restocking shelves, upholding high standards of cleanliness throughout the space, and keeping SWAP signage current and properly displayed.
  • Maintain accurate records of food inventory and submit weekly or as-needed food orders to the Food Program Manager to ensure consistent availability of goods.
  • Maintain accurate and comprehensive case notes, documenting client status, progress, assessments, service engagement, and any relevant correspondence.
  • Record all provided services, referrals, recommendations, case notes, and program documentation into HMIS or Salesforce within 1 business day of client interaction.
  • Complete necessary forms and reports following established procedures.
  • Review and adhere to current grant guidelines to determine client eligibility for services such as rental assistance, utility support, and wraparound subsidies.
  • Ensure shared spreadsheets are updated within 1 business day of any changes.
  • Prepare and submit weekly success stories to supervisor, marketing, and grants manager.
  • Case conference with other case managers internally and externally to TJOP to assist clients in receiving all possible services.
  • Case conference complex cases/issues with supervisor and provide creative solutions to challenges that arise.
  • Attend trainings and meetings to understand social services in our community.
  • Performs other related duties consistent with the scope and purpose of the position.
