The Family Based Mental Health Worker (FBMH worker) will provide Intensive Case Management (ICM) services to help families access resources to meet basic needs. This includes contacting agencies for financial assistance, making referrals for job training, and providing transportation. The FBMH Worker will collaborate with other involved agencies and professionals, such as therapists, school teachers, and CYS caseworkers, at least once a month. They will also make necessary referrals throughout treatment and provide updates on case progress to referral agencies with expressed written consent. Additionally, the FBMH Worker will participate in discharge planning meetings to arrange aftercare services. The role involves co-facilitating Home and Community-based structural family therapy sessions with the FBMH Therapist for a caseload of 8 families. Crisis availability via phone is required when on call, which occurs once every 14 weeks. The FBMH Worker will receive weekly team and group supervision and attend all trainings provided by the Center for Family Based Training. Administrative duties include documenting all client and agency contacts and maintaining productivity requirements.
