About The Position

The Family Based Mental Health Therapist (FBMH Therapist) plays a crucial role in providing Intensive Case Management (ICM) services to families, aiming to help them access resources for basic needs and collaborate with external agencies involved in the client's care. This position involves clinical leadership in family assessments, treatment plan development, and co-facilitating therapy sessions. The therapist is responsible for individual and family treatment, crisis intervention, and ensuring continuity of care, particularly with drug and alcohol treatment facilities. Additionally, the role requires active participation in supervision, ongoing training, and meticulous administrative documentation.

Requirements

A graduate degree in psychiatry, psychology, social work, nursing, education, rehabilitation, or a graduate degree in the field of human services plus 2 years’ experience in a CASSP system program.
OR Be a licensed registered nurse (RN) with 5 years of experience including 2 years of experience in a CASSP system program plus have certification by the Department’s Office of Mental Health (OMH) as a mental health family based worker.
Attend all trainings provided by The Center for Family Based Training and actively progress through training levels (Level I, Level II, Certified Family Based Therapist Exam).
Flexible schedule, including evening hours.
Valid PA driver’s license, auto insurance, and reliable transportation.
May not be employed in another mental health program at Blair Family Solutions or elsewhere.
All necessary child abuse clearances (PA Criminal Record Check, PA Child Abuse Clearance, and Federal Criminal History Clearance).

Nice To Haves

Actively progress through the levels of training, working toward the completion of Level I, Level II, and then the completion of the Certified Family Based Therapist Exam given through the State of PA.

Responsibilities

Provide ICM services to help families access resources for basic needs (e.g., financial assistance, job training referrals, transportation).
Collaborate with other involved agencies at least monthly regarding client/family issues.
Make necessary referrals throughout treatment and provide progress updates to referral agencies monthly and as needed, adhering to confidentiality standards.
Complete drug and alcohol assessments and make referrals to treatment facilities, ensuring continuity of care.
Participate in discharge planning meetings and make necessary aftercare referrals.
Provide clinical leadership in assessing family struggles through intake, timeline, and genogram completion, focusing on strengths and struggles.
Provide clinical leadership in formulating and writing treatment plans with families, reviewing and updating them monthly.
Co-facilitate structural family therapy sessions with the FBMH Worker, taking responsibility for the clinical focus.
Conduct individual and family treatment sessions focusing on issues impacting the family, such as systemic issues, trauma, attachment, anxiety, depression, co-occurring disorders, etc.
Engage families in the treatment process using reflective listening, collaboration, empathy, and goal setting.
Utilize interventions learned through FBMH trainings (e.g., enactments, family sculpture).
Provide weekly consultation with the BHT (if assigned) focusing on treatment plan implementation.
Lead and conduct supervisions for their team.
Provide clinical leadership in developing a 24-hour crisis plan with the FBMH Worker and family.
Provide 24-hour crisis availability on an on-call rotation, including phone counseling and in-person assistance.
Accompany families to crisis centers and complete necessary paperwork for inpatient hospitalization if required.
Ensure a minimum of two sessions (individual, parent, family) per consumer family per week, accommodating family schedules including evenings and weekends.
Provide clinical direction and identify skills for family members to develop, working collaboratively with the FBMH Worker.
Provide drug and alcohol assessment, therapy, referrals, and case consultations.
Complete drug and alcohol assessments for children over 10 with suspected problems and evaluate adults as necessary.
Attend all scheduled supervisions (weekly team and group supervisions).
Attend all trainings provided by The Center for Family Based Training.
Videotape sessions at least twice per family for supervision and clinical trainings, completing treatment adherence scales.
Prepare clinical case presentations with the FBMH Worker for supervision and trainings, including timeline, genogram, clinical assessment, and a videotape.
Document all contacts with consumers and agencies in progress notes within 48 hours, using DAP format and relating notes to the treatment plan.
Complete all sections of progress notes before submission for billing.
Submit progress notes with billing every Monday by 9 am.
Sign off on all billing prior to submission to the Administrative Assistant.
Maintain productivity requirements of 125 units per week with a team percentage averaging 60%.
Ensure encounter forms are signed for every contact (shared responsibility with FBMH Worker).
Collaborate with the FBMH Worker to complete all required treatment plans and reviews on time.
Collaborate with the FBMH Worker to schedule and attend all ISPT Meetings, submitting documentation to the appropriate MCO.
Ensure all documentation is in the chart for specified cases and participate in periodic audits.
Work collaboratively with the FBMH Worker to resolve internal conflicts through direct communication, escalating to the FB Director or Clinical Supervisor if unresolved.
Utilize intentional team building during supervisions to enhance collaboration.
