Family Assessment Worker - Brooklyn - Spanish Required
About The Position
Family Health Centers at NYU Langone is seeking a Family Assessment Worker in Brooklyn. This role involves conducting outreach and screening to identify pregnant or newly parenting caregivers eligible for home visiting services. The successful candidate will manage a caseload of families, aiming to foster positive parent-child bonding, promote child and family health and development, and enhance family resilience. These home-visiting services are delivered within a healthcare system, in collaboration with internal and external stakeholders, and report to the Program Supervisor.
Requirements
- High School diploma/GED
- Experience working with, or assisting at risk children and families in a community setting
- Bilingual Spanish
- Ability to effectively communicate with all levels of the organization

- Infant mental health endorsement preferred (if available in the state)
- Ability to observe and report accurately on the functioning of individuals and families
- Ability to establish trusting relationships and work effectively with mothers, fathers and extended family
- Successful parenting and/or child care experience

- Emotionally mature and capable of exercising judgement

Responsibilities
- Conduct outreach and screening to identify families eligible for home visiting services.
- Initiate and maintain regular contact with families, primarily via visiting them in their homes.
- Document initial family assessments and use assessment tools as indicated.
- Interview parents, preferably prenatally and/or immediately after the birth of their child.
- Review Hospital or Clinic records.
- Provide timely reporting and case documentation in an electronic database management system.
- Assist in making and attending health and human service appointments, including activities related to employment and educational goals.
- Assist parents to improve their skills to optimize the home environment for the child's well-being and safety.
- Assist to strengthen parent-child relationships.
- Provide prenatal and other health education.
- Establish a trusting relationship with families.
- Participate in community events to promote Healthy Families services.
- Establish partnerships with internal and external agencies that work with families.
- Perform other related duties as assigned.
