About The Position

The Familiar Face Case Manager is a behavioral health technician or behavioral health professional specializing in the care coordination and discharge planning of individuals utilizing crisis services at disproportionately high rates. The patient population is determined by individuals who are some of the highest utilizers of crisis services. The Familiar Face Case Manager’s objective is to help prepare the patient for the most clinically appropriate services, assist the patient in obtaining those services, coordinating care with all involved parties, and reducing barriers to effective treatment.

Requirements

High school diploma or GED required.
Minimum of six (6) months of experience working in multiple settings and levels of care, including both mental health and substance abuse is required.
Current Driver’s License (valid and in good standing).
39-month Motor Vehicle Record (MVR).
Fingerprint Clearance Card required.
Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures.

Nice To Haves

Associates Degree in a field related to behavioral health preferred.
Case Management experience, preferred.
One (1) or more years of experience preferred.
Minimum of six months of recovery from substance use and/or mental health disorders preferred.
Peer Certification (if applicable)
