The Facilitator ensures fidelity to the Wraparound model by coordinating and overseeing services for youth and families. The Facilitator promotes a strong family and youth voice in service planning and goal development, ensuring services are individualized, family-driven, flexible, community-based, and collaborative across systems. Serving as the primary point of contact, the Facilitator coordinates care, leads the Wraparound process, and monitors implementation of the individualized Plan of Care. They maintain team cohesion, gather ongoing input from families and providers, and ensure quality service delivery, including crisis intervention and stabilization. The Facilitator implements all four phases of Wraparound-Engagement, Plan Development, Implementation, and Transition-while upholding Wraparound principles and coordinating services across systems such as mental health, education, child welfare, juvenile justice, and tribal systems when applicable.
