About The Position

Easterseals PORT Health (ESPH) is seeking a full-time Wraparound Facilitator in the Mt Airy, NC area to join their caring, community-focused team. The role involves empowering individuals and strengthening communities by delivering services that help youth and families overcome challenges and build brighter futures. Requirements

• Bachelor’s degree in Social Work, Psychology, or related Human Services field
• Must meet the NC Qualified Professional requirements to include education and experience with the population served
• A valid driver's license, current auto insurance and a reliable vehicle for work-related travel

Responsibilities

• Build and lead the Child and Family Team (CFT) to support youth in staying "at home, in school, and out of trouble."
• Coordinate team activities to achieve the goal of keeping youth "at home, in school, and out of trouble."
• Provide direct support and education to families and team members.
• Guide the family in accomplishing objectives outlined in the Family Plan using a strengths-based approach.
• Ensure accurate documentation in the UNCG database.
• Embody the Wraparound philosophy of "Whatever it takes" to meet the unique needs of youth and families.

Benefits

• Competitive hourly salary: $50,000-$55,000, ($24.00-$26.44 per hour)
