About The Position

Easterseals PORT Health (ESPH) is seeking a full-time Wraparound Facilitator in the Mt Airy, NC area to become an essential part of a caring, community-focused team. This role is crucial in empowering individuals and strengthening communities by delivering services that help youth and families overcome challenges and build brighter futures. The organization has been recognized as Raleigh’s Best Nonprofit Organization and Raleigh’s Best Mental Health Services for two consecutive years. As a QP Facilitator on the High-Fidelity Wraparound team, the individual will build and lead the Child and Family Team (CFT) to support youth in remaining at home, in school, and out of trouble. This involves coordinating team activities, providing direct support and education to families and team members, and using a strengths-based approach to guide families in achieving objectives outlined in the Family Plan. Accurate documentation in the UNCG database is required. The role embodies the Wraparound philosophy of “Whatever it takes” to meet the unique needs of youth and families, aiming for meaningful, lasting success.

Requirements

• Bachelor’s degree in Social Work, Psychology, or related Human Services field
• Must meet the NC Qualified Professional requirements to include education and experience with the population served
• A valid driver's license
• Current auto insurance
• A reliable vehicle for work-related travel

Responsibilities

• Build and lead the Child and Family Team (CFT) to support youth on a mission to stay “at home, in school, and out of trouble.”
• Coordinate team activities to achieve the goal of keeping youth at home, in school, and out of trouble.
• Provide direct support and education to families and team members.
• Guide the family in accomplishing objectives outlined in the Family Plan using a strengths-based approach.
• Ensure accurate documentation in the UNCG database.
• Embody the Wraparound philosophy of “Whatever it takes” to meet the unique needs of youth and families.

• Competitive hourly salary: $50,000-$55,000, ($24.00-$26.44 per hour) for this non-exempt position
