About The Position

The Extended Foster Care Review (FCR) Specialist reports to the Extended Foster Care Review Supervisor, and is responsible for coordination/facilitation of the administrative review panel for young adults in extended foster care. In addition, this position ensures young adults in the extended foster care program have the support, services, and quality case management needed to successfully transition to adulthood. This position models the Youth Thrive culture and philosophy when working with young adults and Extended Foster Care Coaching Specialists.

Employee must obtain and maintain clearance of the DCS Central Registry.
Employee must obtain and maintain an Arizona Fingerprint Clearance Card.
A valid Arizona driver's license is required.
This position requires driving or the use of a vehicle as an essential function of the job to conduct State business.

Nice To Haves

Bachelor’s degree from an accredited college or university in education, social work, psychology, counseling or other related fields with 3-5 years of experience working directly with young people in Arizona's foster care system (Master’s degree from an accredited college or university in one of the above mentioned fields will substitute for one year of experience)

Responsibilities

Schedules and completes EFC Case reviews with young adults, their supports, and EFC Coaches.
Charts and identification of tasks needed to support the young adult.
Researches the case prior to review, competes pre-review staffing as needed and completes a comprehensive report on young adults' progress and uploads it to Guardian.
Tracks and inputs programmatic data at each review and documents transition plans are updated every 180 days.
Monitors and reviews deadlines to assure EFC Reviews are completed timely based on federal requirements.
Conducts 90-day contact with young adults to confirm follow-up tasks are completed as intended as documented in the report.
Provides presentations to EFC Coaching staff to promote understanding of the EFC Review process.
Supports the training and mentoring of new EFC Reviewers through the on-boarding process.
Participates in continued training.
Facilitates QIC-EY training to EFC Coaching staff.
Travels to community locations to facilitate reviews.
Other duties as assigned as related to the position.
