About The Position

Help patients safely improve their cardiovascular health, fitness, and overall well-being through individualized exercise programs. Collaborate with Cardiac Rehab clinicians, physicians, nurses, and other members of the care team to support patient goals and outcomes. Conduct fitness assessments and develop exercise prescriptions using ACE, AACVPR, and ACSM guidelines. Provide education, encouragement, and ongoing support to help patients stay engaged and compliant with their prescribed programs. Serve as a knowledgeable resource for exercise programming for cardiac rehabilitation, at-risk adults, and wellness populations. Maintain a safe, organized, and patient-focused environment for cardiac rehabilitation patients and fitness center members.

Requirements

  • Bachelor's degree in Exercise Science, Exercise Physiology, or a related field required.
  • BLS certification required within 30 days of employment and before providing direct patient care.
  • Strong knowledge of ACSM, ACE, and AACVPR exercise guidelines.
  • Ability to conduct fitness assessments and develop safe, individualized exercise prescriptions.
  • Strong communication and interpersonal skills with the ability to work effectively with patients, families, physicians, and healthcare professionals.
  • Strong organizational, critical-thinking, and independent judgment skills.
  • Ability to accurately document patient information and treatment plans in an EMR.
  • Ability to meet the physical demands of the role, including prolonged standing and walking, assisting with patient movement, and lifting or carrying equipment and supplies up to 25 pounds.
  • Commitment to patient-focused care and the organization's mission, vision, and values.

Nice To Haves

  • Master's degree in Exercise Science or a related field preferred.
  • ACSM certification and clinical certification or equivalent preferred.
  • At least one year of continuous experience in a telemetry or related cardiac clinical setting preferred.

Responsibilities

  • Review patient health histories and conduct assessments to develop individualized exercise programs.
  • Prescribe and modify exercise programs based on patient needs, clinical guidelines, and response to exercise.
  • Monitor vital signs and patient responses using equipment such as blood pressure cuffs, pulse oximeters, and glucometers.
  • Educate patients and families about exercise, treatment procedures, safety, and home exercise programs.
  • Collaborate with physicians, nurses, and interdisciplinary team members to communicate patient status and coordinate care.
  • Complete individualized treatment plans and documentation accurately and timely in the EMR.
  • Evaluate exercise outcomes and make appropriate adjustments to support patient progress and safety.
  • Identify potential safety concerns and maintain compliance with infection control and departmental policies.
  • Make appropriate referrals and perform follow-up calls as indicated.
  • Support a patient-focused culture while contributing to the daily operations and success of the Cardiac Rehab program.
