Our programs focus on preventing homelessness, providing shelter and transitional housing, supporting children experiencing homelessness, and helping families secure stable housing. The Homelessness Prevention Case Manager engages families seeking services under the homelessness prevention program. This role involves conducting intakes, assessing participant needs (including rent arrears and future rent), and determining program eligibility in collaboration with the Eviction Prevention Coordinator. The Case Manager will work with internal teams, external service providers, and property owners to provide resources and financial assistance to help participants maintain housing. Additionally, the role requires accurate file setup and documentation in various systems like Data Verse, Salesforce, and the One system.
