About The Position

At Hamilton Families, our mission is to end family homelessness in the San Francisco Bay Area. Since 1985, we have been meeting the most urgent needs of unhoused families in our community. Our programs specialize in preventing family homelessness when possible, providing shelter and transitional housing to families in times of need, supporting the wellbeing and academic achievement of children who experience homelessness, and quickly returning families to safe and stable housing throughout the Greater Bay Area. Requirements

  • A minimum of 2 years of experience in a relevant profession OR Bachelor’s degree from an accredited college or university (social work, psychology, or related field preferred).
  • Knowledge of community resources for families with very low incomes in the Bay Area.
  • Commitment to Hamilton Families’ mission of ending family homelessness in the San Francisco Bay Area.
  • Ideal candidates are familiar and committed to the principles and practices of housing first, participant-centered care, harm reduction, and participant confidentiality.
  • Excitement about working on diverse teams and collaborating with colleagues and participants from many different backgrounds.
  • Ability to navigate sensitive conversations and communications in a judgement-free, honest, and kind manner.
  • Strong work ethic and ability to work independently. Ideal candidates are self-motivated, organized, and clear communicators who are comfortable following program policies and processes while delivering high-quality outcomes within urgent timeframes.
  • Flexible and creative; open to creating and joining a culture of feedback and learning; ability to meet multiple deadlines and priorities with a positive attitude and problem-solving approach.
  • Comfort with change and ambiguity is a must.
  • Detail-oriented and demonstrated experience keeping thorough notes and records.
  • Proficient in Microsoft Office (Word, PowerPoint, Outlook, Excel, etc.).
  • Criminal background check, fingerprint imaging, and tuberculosis (TB) clearance required post-offer.
  • Bilingual positions require verbal and written proficiency in English as well as the second language for which the bilingual premium applies, as demonstrated through agency-administered proficiency testing.

Nice To Haves

  • Knowledge and experience with housing laws is a plus.
  • Experience using Salesforce.com and/or SharePoint, One System, Data Verse databases a plus.

Responsibilities

  • Engage families who are seeking services under the homelessness prevention program.
  • Conduct intakes and assess participants’ needs, including reviewing total months owed for back rent and/or future rent and goals to maintain housing stability in their unit.
  • Determine participants’ eligibility into the program and assess total funds granted to families for back rent and/or future rent in collaboration with the Eviction Prevention Coordinator.
  • Work in collaboration with internal teams, as well as external service providers and property owners through resources and financial assistance to aid participants in maintaining housing during and completion of program.
  • Ensure that files are set up accurately with all required documentation in Data Verse, Sales Force, and the One system if applicable.
  • Review vulnerability assessment scores of incoming referrals for families deemed eligible for program per the Homelessness Prevention Platform.
  • Provide intake assessment support for family cases deemed eligible for program, which includes requesting vital documentation of case family members.
  • Engage with property owners to request their property W-9 and proof of total rent owed from family. Work along with The Real Estate department to ensure property owners’ documents are legible and accurate. Ensure documentation is clear making sure to document efforts made to obtain cooperation of property owners. Work to obtain cooperation of landlords in the process for smooth transition of funds.
  • Ensure any payments disbursed on a family’s behalf are concise with contractual obligations.
  • Develop and implement a Housing Stability plan in collaboration with the household.
  • Provide budgeting and money management assistance and connection to related services that support housing stabilization.
  • Provide resources and eligible referrals to community-based organizations to ensure families will remain stable in their unit during and upon completion of program. Referrals such as housing/tenant counseling, credit and budget management are encouraged.
  • Assist families and individuals to establish housing stability goals and identify appropriate steps towards achieving goals including modifying goals as needed.
  • Coordinate and collaborate effectively with other service providers as needed to assist clients to reach their goals through available community resources.
  • Maintain participant confidentiality and HIPAA requirements.
  • Maintain precise and accurate documentation of case management services, including participant files and entries into various participant databases.
  • Prepare reports and presentations as required.
  • Reinforce and follow appropriate boundaries with participants.
  • Ensure operational excellence of department, including preparation for database and participant hard file audits.
  • Other duties as assigned.
