About The Position

The Evening Supervisor/RN or LPN is responsible for assisting the Memory Care Director (MCD) with the on-going operations of the Memory Care Department. This role ensures the highest degree of quality care is maintained at all times and manages the department in accordance with Executive Office of Elder Affairs (EOEA) regulations when the MCD is unavailable. The position promotes an environment that respects and ensures Residents’ rights. The Village at Proprietors Green (VPG) is a luxury apartment senior living community offering independent, assisted living, and memory care apartments.

Requirements

  • RN or LPN licensed in the state of Massachusetts.
  • Knowledge of geriatrics, Alzheimer’s disease or related disorders.
  • Excellent interpersonal, organizational, verbal, written and computer skills.
  • Willingness and ability to work as a collaborative team member.

Nice To Haves

  • Computer skills preferred.
  • Experience in a clinical setting including skilled settings, home-care settings, VNA, but preferably an assisted living environment.

Responsibilities

  • Perform Initial assessments if the MCD is not available.
  • Assist MCD with training and supervision of all Memory Care nursing staff.
  • Update assessments as required.
  • Develop Service Plans and updates as required by EOEA Regulations.
  • Develop and conduct Introductory visits as needed.
  • Admit new residents-includes completing all required paperwork and distribution of documents.
  • Assist with resident transition to and from the assisted living community.
  • Assures that policies and procedures are routinely and consistently followed.
  • Assures that all documentation for Resident records is up to date for medication documentation, ADL documentation and contact information.
  • Performs other tasks as requested where hands on intervention or participation are required.
  • Keep 24-hour notes current.
  • Check medications after a delivery to assure accuracy.
  • Assist with Maintaining the emergency list of AL residents.
  • Communicate with other departments information pertinent to maximize success with new Resident or current Residents.
  • Assures Resident’s rights.
  • Maintains confidentiality.
  • Input for annual staff performance reviews resident assistants.
  • Assist with the Safety, SAMM and Fall QA Programs as regulated by EOEA.
  • Complete all required yearly in-services.
  • If needed assist with initial EOEA staff orientation.
  • Liaison to VNA, pharmacies, families, MD’s and all other providers.
  • If applicable, participate in family meetings.
  • Document and report incidents-completing those that need to be reported to EOEA.
  • Calls to families/MD’S and other pertinent people in response to issues or incidents.
  • Contribute daily communication for RA’S and nurses.
  • Contribute comprehensive notes for all resident care staff nurses, SCU Program Director, AL Clinical Consultant and Social Worker.
  • Oversees emergency response procedures and respond to medical emergencies.
  • Works a self-directed schedule that may include weekends, holidays and or evenings.
  • On-call as scheduled.
  • Provide resident visits-document findings and follow-up.
  • Assist as needed with RA skills and SAMM observations as scheduled.
  • As needed-Keep resident assistant assignments updated as needed.
  • As needed-Update / I’m OK/Diet lists.
