About The Position

The ERE Outreach Specialist is responsible for providing assertive engagement to individuals experiencing mental health or substance use symptoms. This role involves working with a team to assist individuals in gaining access to needed psychiatric, medical, substance use services, social, residential, financial, and other appropriate services. The specialist will network with community partners to deliver an enhanced safety-net and work in non-traditional ways to meet client needs and improve health and well-being. The Crisis Intervention Program focuses on outreach services for individuals at risk of harming themselves or others, involving nonjudgmental assessments to ensure safety and determine treatment disposition. The Community Counseling Center (CCC) provides these services in conjunction with the Department of Mental Health through the Missouri Access Crisis and Response System (MOCARS).

Requirements

  • Bachelor's degree in human service field, or combination of qualifying experience and education; or qualifying experience.
  • Two years of direct service to the target population, or other related human service experience.
  • Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents), and proof of continuous auto insurance.
  • Must also meet CCC's Motor Vehicle Record standards policy and have reliable transportation.

Responsibilities

  • Complete required assessments.
  • Complete community outreach, including routine home visits for those who are high utilizers of services.
  • Interact in a non-traditional way to develop rapport to meet them where they are to engage them into the services that best fit their individual needs.
  • Build and maintain an extensive knowledge of community resources and have the ability to match person with eligible resources.
  • Gain client access to services with creative attempts to help identify and obtain resources and needed services. This includes a strong role as an advocate for the person.
  • Maintain and initiate clear, concise communication that is timely and accurate with all referral sources involved in the service provision.
  • Monitors service delivery by maintaining regular contact with person and service providers/ programs involved.
  • Complete follow-up calls, maintain contact for increased success.
  • Help the person increase his/her support network outside of the behavioral health service delivery system.
  • Participate in the on-call schedule rotation for after-hours crisis calls.
  • Responsible for transporting clients in agency and/or personal vehicle as deemed necessary and appropriate to meet program requirements.
  • Document according to clinical policies and procedures, DMH, CARF, and any other federal or state program per agency standards.
