About The Position

The Enrollment / UAS Nurse is responsible for conducting pre-enrollment assessments for prospective enrollees, leveraging clinical nursing expertise and critical thinking skills, including gathering preventive health service data. This role involves completing the UAS - New York Assessment instrument to determine ongoing eligibility and identify necessary adjustments to Care Plans. The nurse will skillfully assess, coordinate, and evaluate consumer needs, and develop prospective plans of care. Communication with the Care Team and participation in interdisciplinary team meetings are integral to the position. Assessments will be performed in Westchester and require reliable transportation, as well as conducting environmental assessments. The nurse will also complete semi-annual assessments as requested and must maintain a flexible schedule to respond to unexpected emergencies and changes in workload. Chart audits may be conducted if time permits.

Requirements

• Current NY State Registered Nurse License
• Bachelor’s Degree in Nursing

Responsibilities

• Utilizes a skillful approach for assessing, coordinating, and evaluating consumer needs and will need to develop a prospective plan of care.
• Communicates with the Care Team and participates in interdisciplinary team meetings.
• Assessments will be conducted in Westchester. Reliable transportation required.
• Responsible for environmental assessments.
• Completes semi-annual assessments as requested
• Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities.
• Conducts chart audits as requested if time permitting
