About The Position

This role involves conducting pre-enrollment assessments for prospective enrollees using clinical nursing expertise and critical thinking skills. The nurse will also complete the UAS - New York Assessment instrument to determine continuing eligibility and identify any necessary changes to the Care Plan. A key aspect of the role is assessing, coordinating, and evaluating consumer needs, and developing prospective plans of care. The position requires communication with the Care Team and participation in interdisciplinary team meetings. Assessments will be conducted in Westchester, requiring reliable transportation. The role also includes environmental assessments and semi-annual assessments as requested. Flexibility in scheduling and responsiveness to emergencies and workload changes are essential. Chart audits may be conducted if time permits.

Requirements

• Current NY State Registered Nurse License
• Bachelor’s Degree in Nursing

Responsibilities

• Conducting pre-enrollment assessments for prospective enrollees utilizing clinical nursing expertise and critical thinking skills including obtaining preventive health service data.
• Completes UAS - New York Assessment instrument to determine continuing eligibility and / or identify any needed changes to Care Plan.
• Utilizes a skillful approach for assessing, coordinating, and evaluating consumer needs and will need to develop a prospective plan of care.
• Communicates with the Care Team and participates in interdisciplinary team meetings.
• Assessments will be conducted in Westchester. Reliable transportation required.
• Responsible for environmental assessments.
• Completes semi-annual assessments as requested.
• Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities.
• Conducts chart audits as requested if time permitting
