Enrolled Nurse - Pt Broughton (TCP Coordinator)
About The Position
The Enrolled Nurse - TCP Coordinator supports the delivery of person-centred care through the Transition Care Program (TCP) at Port Broughton. The role assesses client suitability, coordinates admissions, establishes individual goals, and develops plans that promote independence and a successful return to the home environment. The Enrolled Nurse works collaboratively with patients, families, allied health professionals, and the wider care team. With support from the Nursing Unit Manager, the role also develops and implements staff resources and education to support delivery of the TCP across a 24-hour period.
Requirements
- Be enrolled, or eligible for enrolment, as a Nurse with the Nursing and Midwifery Board of Australia.
- Hold, or be eligible to hold, a current practising certificate.
Responsibilities
- Undertake suitability assessments and coordinate admissions to the health service.
- Work with patients to establish goals and plans that promote independence.
- Support home visits and family conferences.
- Coordinate allied health services during admission and in preparation for patients returning home.
- With support from the Nursing Unit Manager, develop and implement staff resources and education to support delivery of the TCP across a 24-hour period.
