POSITION OVERVIEW: Title: Enhanced Care Management Coordinator Reports To: Tribal Registered Nurse Schedule: M-F; 8am-5pm Status: Non-Exempt Location: Tuolumne Primary Summary: The Enhanced Care Management (ECM) Coordinator supports the delivery of enhanced care management services for high-need Medi-Cal members served by TMWIHC. Reporting to the Tribal Registered Nurse, this position coordinates comprehensive care plans, facilitates interdisciplinary care team collaboration, and connects members with community resources to address social determinants of health and improve health outcomes.

Requirements

  • Pattern I: Bachelor’s degree in social work, nursing, public health, psychology, or a related field, plus one (1) year of care coordination or case management experience.
  • Pattern II: Associate’s degree plus three (3) years of experience in care coordination, case management, or community health work.
  • Valid California Driver’s License and personal automobile insurance meeting TMWIHC minimum coverage requirements.
  • Must be comfortable conducting home and community visits.
  • This position requires walking, standing, and sitting for prolonged periods; climbing stairs and step-stools; stooping, kneeling, crouching, balancing, and reaching; pushing, pulling, and lifting up to 30 pounds; and raising objects overhead.
  • The role also requires manual dexterity for operating computers and phones, as well as hearing, talking, and strong verbal and written communication skills.
  • Repetitive motions of the wrists, hands, and fingers are involved.
  • The employee may be subject to outdoor environmental conditions such as heat and cold, and will be required to walk on uneven ground.
  • The position also requires the ability to safely operate and drive a motor vehicle.
  • All positions require cultural sensitivity and the ability to interact respectfully and effectively with diverse tribal communities.
  • Employment is contingent upon successful completion of a pre-employment TB test, background check, and drug screening.

Nice To Haves

  • CCM (Certified Case Manager), LSW, or equivalent certification preferred.
  • Knowledge of Medi-Cal Enhanced Care Management (ECM) and Community Supports (CS) program requirements.
  • Experience working with Native American populations or underserved communities.

Responsibilities

  • Serve as the primary care coordinator for high-need Medi-Cal members enrolled in the Enhanced Care Management program.
  • Conduct comprehensive needs assessments and develop individualized care plans in collaboration with the patient and care team.
  • Coordinate services across medical, behavioral health, social services, and community support domains.
  • Connect members with community resources and social services to address housing, food security, transportation, and other social determinants of health.
  • Maintain regular contact with assigned members through in-person visits, phone, and telehealth.
  • Document all care management activities accurately and timely in the electronic health record.
  • Participate in interdisciplinary case conferences and collaborative care meetings.
  • Monitor member progress and adjust care plans as needed.
  • Ensure compliance with Medi-Cal ECM program requirements, documentation standards, and reporting obligations.
  • Perform other duties as assigned.
