About The Position

Chartiers Center is a private, non-profit corporation funded by state, county, third party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program. The Janus Program is a new and exciting model of care that uniquely assists participants to liven on their own and learn desired skills. The Enhanced Case Manager will provide case management services by coordinating benefits, treatment and support resources for participants.

Requirements

Bachelor’s degree in human services and 3 years of mental health experience OR Master’s degree in human services with 2 years of experience
Valid Driver’s License Required

Responsibilities

Coordinate and manage the delivery of behavioral health, medical social service and community support services.
Assess participant needs and develop individualized, person-centered service plans.
Meet with participants on a frequent basis to provide ongoing case management interventions and monitor participant progress.
Maintain accurate and timely documentation in compliance with organizational and regulatory requirements.
Participate in rotating on-call responsibilities to provide after hours support for Janus program participants.
