About The Position

Chartiers Center is a private, non-profit behavioral health care organization serving Southwestern Pennsylvania. Our mission is to provide compassionate support to those in need who face behavioral health, co-occurring disorders or intellectual/developmental disability (IDD) challenges as they transform our lives. As a Blended Service Coordinator with Chartiers Center, you would provide support and service coordination to adults living with serious mental illness and co-occurring conditions, including substance use disorders, physical health concerns and intellectual disabilities. This is a highly rewarding community-based role where you build meaningful relationships, advocate for individuals and help them achieve greater stability, independence, and recovery.

Requirements

  • Associate’s degree in human services and 3+ years of mental health experience.
  • Bachelor's degree in human services and 2 years of mental health experience OR Master’s degree in human services with one year of experience
  • Valid Driver’s License Required
  • Ability to travel throughout the community

Responsibilities

  • Meeting with individuals in homes and community settings.
  • Conduct assessments and identify strengths and needs and goals.
  • Develop, implement and monitor individualized service plans.
  • Connect individuals with housing, benefits, healthcare, behavioral health treatment, employment and community resources.
  • Coordinate services among healthcare providers, therapist, family members and community agencies.
  • Maintain regular communication with individuals to monitor progress and identify emerging needs.
  • Advocate on behalf of individuals across multiple systems.
  • Maintain timely and accurate documentation.
  • Participate in an on-call rotation, typically no more than once per month.
