Requirements
- Resume (with reason for leaving each job. If you don't include that, then fill out the work history section)
- Two Letters of Recommendation
- A valid copy of your teaching credential
- Mandatory Employment Disclosure section MUST be complete
- Driver's license or valid ID listed on I-9 form
- Copy of current negative TB test result (Less than 4 yrs old, no risk assessments will be accepted)
- Fingerprints
- Copy of signed Social Security Card
