About The Position

The Engagement Specialist will work closely with hospitals and other external agencies to coordinate care and resources for individuals in Merakey programs. This position will focus on developing relationships with hospital inpatients and emergency departments, residential settings, and treatment facilities, in addition to external resources to meet social determinants of health needs of individuals in services. This individual will attend community, internal, and stakeholder events to promote the programs and generate referrals. This individual will also work with consumers individually to assist in completing and collecting referral information to link them to a Merakey program or other resources within the community.

Requirements

• A bachelor’s degree in chemical dependency, sociology, social welfare, psychology, nursing, or a related field; OR a successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice, or other related social sciences
• A clear criminal background check and Child Abuse clearance
• Basic computer skills
• A clean, valid driver’s license and personal transportation for travel within Delaware County
