About The Position

This is a full-time position for an Inpatient Endocrinology APP (NP/PA) at the Adams Neurosciences Center at St. Raphael in New Haven, Connecticut. The role operates on a Monday-Friday schedule from 8 am to 5 pm with no on-call responsibilities. The position is within the Yale New Haven Health System, which includes multiple hospitals and a physician group. The Adams Neurosciences Center, opening in 2027, will be a state-of-the-art facility focused on advanced neurological and neurosurgical care. The APP will be an integral part of an interdisciplinary team, providing comprehensive patient care from diagnosis to prognosis, working collaboratively with attending physicians and adhering to hospital policies and protocols. The role involves obtaining medical histories, performing physical examinations, ordering diagnostic tests, managing patient treatment plans, prescribing medications, and potentially assisting in operative cases. The APP will also be responsible for patient education, psychosocial support, accurate documentation, and maintaining high standards of ethical behavior and professionalism. Continuous professional development and engagement in quality improvement projects are expected.

Requirements

Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA) OR Connecticut State Registered Nurse License, Connecticut State Nurse Practitioner License, Board specialty certification, as appropriate for position profile.
Connecticut State Physician Assistant License OR Connecticut State Nurse Practitioner License.
ACLS/BLS Certified, when indicated for position profile.
DEA License, when indicated for pediatric position profile.
PALS, when indicated for neonatal care.
NRP Certification.
APRNs, PAs, CRNAs and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process.
Evidence of current competence to practice as an APRN, PA, CRNA, CNM via appropriate reference letters from physicians and other practitioners must be able to be obtained during the Medical Staff appointment process in order to be eligible.

Nice To Haves

Clinical working experience as a NP/PA within the given specialty/service line recommended.
Hands-on experience managing patients on insulin, including real-time adjustment of dosing in response to blood glucose fluctuations.
Ability to assess and incorporate patient comorbidities when modifying insulin therapy and overall treatment plans.
Experience with insulin pump therapy, including prescribing, titrating, and making ongoing adjustments to insulin regimens.
Comfort managing complex diabetic patients and independently making clinical decisions related to insulin management.
Experience caring for hospitalized patients with diabetes and endocrine disorders in an acute care setting.
Familiarity with continuous glucose monitoring (CGM) technology and interpretation of glucose data trends.

Responsibilities

Provides assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team.
Provides optimal patient care, from diagnostic to prognostic matters, in collaboration with attending supervising/collaborating physician(s).
Obtains medical histories, performs physical examinations, reviews/orders diagnostic/therapeutic tests and radiologic results, formulates and manages patient treatment plans.
Performs procedures, prescribes treatment and medications in accordance with institutional and state guidelines.
May function as first or second assistant on operative cases.
Educates patients/families on care and recommended or available resources, including admitting instructions, treatment plans, side effects of treatments/medications, and discharge plans.
Provides psychosocial support to patients, their family members, and significant others.
Ensures appropriate documentation in accordance with hospital and regulatory requirements.
Demonstrates a high standard of moral and ethical behavior, professionalism, compassion, and commitment to patient care.
Ensures compliance with patient-related healthcare regulatory requirements, standards, and recommendations.
Continuously engages in projects that maintain and advance professional competency.
Serves as a clinical resource for new house staff and may provide service-specific orientation.
Consistently demonstrates high reliability organizational behaviors through reporting safety events as well as near misses.
May serve as a clinical preceptor for NP/PA students and other healthcare professionals.
