Expanding practice seeks an Endocrinologist to work in San Diego, California! They serve over 108,000 patients through a network of 47 program sites and utilize a mission-driven, patient-centered medical home model of care. This role offers the opportunity to supervise nurses and serve as a preceptor for PAs and NPs developing in expanded medical roles.

Requirements

• Graduate of an accredited school of Medicine
• Board Certified or Board Eligible in a primary care specialty
• Current CA MD/DO and DEA license
• CPR certification
• Completion of clinical training

Responsibilities

• Manages and provides acute, chronic, preventive, curative, and rehabilitative medical care
• Experience with transitions of care, chronic disease management, and advanced goals-of-care discussions
• Inpatient skilled nursing coverage and call coverage as assigned by the Medical Director
• Case management of clinic patients admitted to hospitals, with referrals to medical specialties
• Consultative services for complex diagnostic or therapeutic problems as requested
• Interdisciplinary team care planning
