End of Life/Hospice CNA
About The Position
Bay Bluffs- Emmet County Medical Care Facility is seeking nurses with geriatric, end of life and/or palliative care experience to join their team. The facility aims to provide residents with exceptional care in a home-like environment with compassion and dignity. They utilize a team approach, combining dedicated administration with an experienced professional staff to offer a variety of services and programs, including short-term rehabilitation, long-term care, dementia-focused care, and End of Life/Hospice care. They are looking for compassionate, hardworking individuals to join their team of dedicated caregivers. Bay Bluffs is ranked in the top 10% in the country. This specific CNA role will be assigned to the new End of Life/Hospice Suite area. Current shift availability is night shift (5:45 PM to 6:00 AM).
Requirements
- Current state license as a CNA
- Proof of current CPR certification
- Compassionate, friendly demeanor
- Excellent communication skills
- Ability to pass a background check and fingerprint process

Responsibilities
- Complete initial and ongoing assessments to identify the physical, psycho-social, and environmental needs of residents
- Assess residents’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review
- Administer medication as prescribed by the resident’s physician
- Provide effective communication to residents, their family members, team members, and other health care professionals
