About The Position

The Crisis, Outreach, Reduction, and Engagement (CORE) Team Specialist plays a vital role within the Wake County EMS Advanced Practice Paramedic program. CORE Specialists ensure that the highest utilizers of EMS services are identified, connected with appropriate resources and aligned with care that meets their unique needs across the larger healthcare continuum. This dynamic team collaborates closely with divisions throughout the department as well as partner county and external agencies, driving forward Wake County’s strategic vision for community health and Wellbeing. Requirements

• Associates degree in EMS or healthcare related field OR social work or a related field
• Three years cleared as a Paramedic in the Wake County EMS System OR demonstrated experience in clinical social work or an adjacent field
• Certification as a North Carolina Paramedic or eligibility for such certification
• Valid Driver's License and "safe" driving record
• Equivalent education and experience are accepted
• Proficiency in working with Salesforce and/or case management agencies

Nice To Haves

• Bachelor’s degree in Emergency Medical Services or related field OR social work or a related field
• Current experience as an Advanced Practice Paramedic

• Connect, engage and meet with high volume utilizers and other specialty patients to provide patient education directly related to medical conditions and social determinants of health
• Apply advanced knowledge of social determinants of health to identify gaps and barriers affecting a patient's overall quality of life and their need for the EMS System
• Complete documentation through patient care reporting systems, electronic logs, reporting software and databases
• Maintain confidentiality and protect sensitive data, in accordance with HIPAA laws, and organizational and departmental policies and procedures
• Collaborate with community partners to evaluate any specialty treatment and/or care plans for high volume utilizers to reduce EMS and hospital utilization
• Participate in meetings when required with partner agencies and provide professional contributions and feedback
• Respond to EMS calls when requested and evaluate patient conditions based on criteria and advanced decision-making skills and possible alternative transport to an alternative medical care facility
• Draft patient specific destination and treatment plans to align community collaborative efforts
• Facilitate meetings with the Division of Medical Affairs, sharing drafted destination and treatment plans
