About The Position

The Azalea Health Analyst serves as a key member of the Population Health and Value-Based Care team, supporting Accountable Care Organization (ACO) initiatives through the administration, optimization, analysis, and reporting of the Azalea Health Population Health platform. This role partners with clinical, operational, quality, care management, and executive stakeholders to leverage data, improve patient outcomes, identify care gaps, support quality performance programs, and maximize success across value-based reimbursement arrangements.

Requirements

• Experience with the Azalea Health Population Health platform.
• Experience in administration, optimization, analysis, and reporting.
• Ability to partner with clinical, operational, quality, care management, and executive stakeholders.
• Skills in leveraging data to improve patient outcomes.
• Ability to identify care gaps.
• Experience supporting quality performance programs.
• Understanding of value-based reimbursement arrangements.
