The Physician Assistant or Nurse Practitioner is a licensed professional provider who functions in collaboration with an attending physician in a variety of practice settings at YNHHS. Provides exceptional direct diagnostic and therapeutic patient care to the highest standards of the organization as part of a multidisciplinary team. Cultivates a patient centric environment which focuses on the whole individual inclusive of physical, psychosocial, spiritual and functional needs of the patient, family, and significant others. Serves as the focal professional for the continuity of care in patient care within scope of practice standards, as well as specialty specific standards to address and resolve problems that may arise. Full-time (1.0 FTE) Advanced Practice Provider (APP) position supporting Emergency General Surgery and Trauma services in an ambulatory setting. Provide comprehensive patient care including evaluation, diagnosis, treatment planning, and follow-up for patients with acute general surgery and trauma-related conditions. Collaborate closely with surgeons, multidisciplinary teams, and support staff to deliver high-quality patient care. Experience in acute care surgery, general surgery, or trauma is preferred. Practice locations may include, but are not limited to: 330 Orchard St., New Haven, CT (SRC) 800 Howard Ave., New Haven, CT Schedule: Four (4) 10-hour shifts per week. Work hours: 7:00 AM – 5:30 PM. Position may include care coordination, patient education, and participation in quality improvement initiatives.
