About The Position

Columbia Community Mental Health (CCMH) is a nonprofit agency and Certified Community Behavioral Health Clinic dedicated to serving individuals with mental, addiction, and developmental disabilities. The Community Developmental Disabilities Program (CCDP) within CCMH provides access to local, regional, and statewide resources for children and adults with developmental disabilities. This includes comprehensive case management, service coordination, assistance with early intervention, and support during life transitions. The Eligibility Specialist plays a key role in ensuring eligible individuals receive comprehensive and collaborative support. The position focuses on providing community education regarding eligibility criteria, managing prospective new intakes and the application process, determining initial eligibility, and conducting eligibility redeterminations at various developmental stages, all in compliance with Oregon Administrative Rules (OARs). Requirements

  • Minimum Bachelor’s degree in behavioral science, social science, human services, public administration, or a closely related field.
  • One year of experience in the public service system for developmental disability services in Oregon (if Bachelor's degree is in any field).
  • OR three years of professional-level evaluative, human services, analytical, and planning experience.
  • OR any combination of experience and education equivalent to five years of experience that typically supports the knowledge and skills for the position, as approved by the Department of Human Services (DHS).
  • Knowledge of I/DD Services and case management.
  • Skills to interact with clients, family, and subcontracted providers.
  • Good computer skills.
  • Reliable transportation, valid driver’s license, and current auto insurance meeting agency standards.
  • Ability to pass agency MVR test.
  • Must be authorized to work for any employer in the U.S.

Responsibilities

  • Provide community education on eligibility criteria.
  • Complete prospective new intakes and manage the application process.
  • Determine eligibility for services.
  • Conduct eligibility redeterminations at various ages of development.
  • Ensure all processes comply with Oregon Administrative Rules (OARs).
