About The Position

myPlace Health is seeking an Eligibility Coordinator passionate about serving the underserved community in Los Angeles County. This role is crucial in helping frail seniors apply for Medicaid or Medicare, enabling them to continue living at home independently. The ideal candidate will have direct experience with the Medicaid and Medicare new/renewal process and a strong understanding of the California Medi-Cal system.

Requirements

  • Knowledge of the California Medi-Cal System and Social Security process.
  • 2-4 years of relevant experience.
  • Experience working with State/County eligibilities workers.
  • Experience working with the Frail Elderly.
  • Tuberculosis screening upon hire or proof of a negative screening within the last year.

  • Bilingual: Spanish, Cantonese, Mandarin, Vietnamese preferred.

Responsibilities

  • Initiates, assists, and manages Medi-Cal applications for prospective participants, ensuring timely and accurate completion.
  • Conducts home visits to complete the Medi-Cal application process.
  • Assists participants, families, and caregivers in completing necessary paperwork for Medicaid re-determination or new applications.
  • Maintains all documentation and records all enrollment in CRM software, ensuring accuracy and compliance.
  • Strictly adheres to DHCS and CMS regulations and policies regarding marketing practices for the PACE Program.
  • Collaborates with the Finance Department to prepare reports for data analysis and validate Medicaid payments.
  • Monitors and maintains participant's Medicaid eligibility (recertification of Medi-Cal).
  • Supports the Enrollment and Outreach Specialist as needed.
  • Provides community resources to prospective participants or families/caregivers.
  • Collaborates with or contacts county/state government specialists to ensure submission of required documentation or to obtain additional information.
  • Performs other duties as assigned.
