These professionals ensure residents receive the support they need by evaluating eligibility for critical programs that impact health, safety, and long-term stability. This is a hybrid role, allowing for 50% in-office and 50% virtual work, offering flexibility and connection. The position involves determining and maintaining ongoing eligibility for Adult Medicaid Long Term Care, Special Assistance, and/or the Community Alternative Program for Children (CAP-C). Responsibilities include processing applications, managing cases using NCFAST and OnBase, entering accurate and timely data, referring clients to community resources, staying current on policies, providing backup support, contributing to emergency preparedness, and documenting case activity electronically.
