About The Position

Community First Solutions is seeking an Intake Assessor to join their Elderly Services Program (ESP) team in Butler County. This role involves screening referrals, conducting telephone assessments, and connecting seniors with resources and services to help them stay safe, healthy, and independent. The Intake Assessor will be the first point of contact for seniors, caregivers, hospitals, and community partners, guiding them through available services with empathy and professionalism. This position is ideal for individuals with a background in social work, gerontology, nursing, or healthcare who are passionate about making a difference in the lives of older adults and their families.

Requirements

  • Licensed Social Worker OR Registered Nurse OR Bachelor’s prepared degree in social work, gerontology or related field.
  • Acceptable criminal records check as defined in the ODA criminal records check rule 173-14-01.
  • Excellent communication and organizational skills.
  • Proficient in basic computer applications.
  • Valid driver's license, insurance, and reliable transportation.
  • Ability to complete home visits.
  • Must receive AIRS Certification within the first year of employment.

Nice To Haves

  • At least one year of experience in geriatrics, case management, intake/interviewing, long-term care, home health, or medical social work.
  • Appropriate experience may suffice; approval required by program Administrator.

Responsibilities

  • Assess client referrals for program eligibility and enroll eligible individuals according to program policy and procedures.
  • Develop individualized service plans in collaboration with clients and/or caregivers, including community resources and ESP services.
  • Provide information, referrals, and/or resources as needed.
  • Inform clients of their rights and responsibilities.
  • Monitor service plan costs to ensure adherence to cost caps.
  • Secure and monitor services to ensure quality of care, supporting participant health and safety.
  • Maintain the client contact schedule of required interventions (phone or home visits) to assess and verify service plans, identify problems, and assess changes in client status, conditions, or needs.
  • Complete documentation as required per procedure.
  • Maintain ongoing contact and open communication with caregivers, family members, friends, and other sources of informal support as appropriate.
  • Work with clients, service providers, and others involved in the client’s plan of care for mutual problem-solving and care coordination.
  • Complete review of income and medical expenses to determine client co-payment amount, if applicable, and monitor co-payment status.
  • Navigate insurance, Medicare, Medicaid, Advantage plans, make referrals to benefit specialists, act as payor of last resort, and pursue other funding sources.
  • Assist clients/caregivers with discharge planning as appropriate upon disenrollment from ESP and/or from hospital/nursing home.
  • Follow policy and procedure for processing client appeals and participate in appeal proceedings as required; disenroll clients who no longer meet eligibility requirements.
  • Cooperate with the care management team to ensure adequate and appropriate care management coverage of all agency clients.
  • Provide information and referral about the aging network, services, and other community resources to callers.
  • Complete telephone assessments for the Elderly Services Program and send new enrollments to the designee for case assignments.
  • Enter screening information into the designated software system per procedure.
  • Maintain records and complete reports as needed or requested.
  • Assist with online referrals and/or questions submitted via the website.
  • Maintain open lines of communication with hospitals, community agencies, and other referral sources.
  • Assist with the maintenance of the internal resource database.
  • Maintain accurate and confidential computer records of client activity using care management software.
  • Adhere to confidentiality policy and HIPAA protocol related to all client information/interactions.
  • Follow up on reported incidents and health and safety issues, completing incident reports and/or complaint forms as necessary.
  • Participate in the PQI process as directed by supervisor or manager, including peer audits and supervisory chart audits.
  • Manage timekeeping.
  • Promote a positive community image at all times.
  • Attend required program and agency trainings and meetings.
  • Participate in case conferences.
  • Attend professional meetings, workshops, and seminars as appropriate.
  • Perform all other duties as assigned and/or appropriate to the position.
