About The Position

The Educational Support Services Coordinator provides comprehensive youth and family support services focused on improving school attendance, increasing community engagement, strengthening family resilience, reducing barriers, and creating pathways toward education, employment, and long-term stability. The Coordinator provides individualized case management, Healthy Lifestyle Assessments, goal planning, empowerment programming, caregiver support, youth development activities, and connections to workforce pathways and community resources. Physical demands are consistent with work performed in a standard office environment. This job description outlines the general scope of responsibilities and does not constitute an employment contract, expressed or implied, other than an at‑will relationship. The Director of Supportive Services may modify or assign duties at any time. To perform this job successfully, you must be able to carry out each essential job function satisfactorily.

Requirements

  • Bachelor's degree in social work, psychology, sociology, or a closely related field from an accredited four-year college or university
  • Minimum two (2) years of case management experience, or experience delivering youth development/OST programs.
  • Minimum one (1) year working with youth and young adults from various ethnic and cultural backgrounds, ages, as well as economic circumstances.
  • Experience facilitating groups, workshops, or culturally responsive youth/family programming.
  • Knowledge of trauma-informed, strength-based, and culturally responsive practices.
  • Ability to develop partnerships and navigate community resources.
  • Willingness to learn a variety of tasks; possesses excellent communication skills, a thorough understanding of nonprofit practices, a serious work ethic, and a desire to work in a team-orientated environment.
  • Possesses the ability to work independently, establish priorities, and meet deadlines.
  • Demonstrate skills in conflict management.
  • Competency in MS Office: Word, Access, Publisher, Outlook, Excel, and PowerPoint.
  • Strong organizational and communication skills with the ability to manage multiple priorities.
  • Ability to maintain a good working relationship with all co-workers and the public.
  • Ability to travel to within 15-mile radius of the office.
  • Strong knowledge of workforce development best practices that include demonstrated ability to prepare participants for employment through educational opportunities, certifications or other vocational programs that lead to viable employment opportunities.
  • Must have knowledge of community, educational and economic resources and social services in South Minneapolis.
  • Valid MN Drivers’ License, access to reliable transportation and the ability to meet Sabathani’s driving record requirements.

Responsibilities

  • Lead recruitment, outreach, engagement, and home visits for students and families.
  • Maintain an active caseload and provide ongoing 1:1 case management.
  • Complete Healthy Lifestyle Assessments, identify barriers, and develop individualized goals.
  • Connect participants to resources that improve stability, attendance, employment readiness, and wellbeing.
  • Support improved school attendance, educational engagement, behavior improvement, and future planning.
  • Coordinate empowerment programming focused on social-emotional skills, leadership, safety, belonging, resilience, healthy relationships, and hope for the future.
  • Facilitate caregiver/parent support programming designed to increase family resilience, advocacy skills, parent involvement, and social connection.
  • Provide out-of-school-time activities, youth enrichment opportunities, workshops, and prosocial programming.
  • Provide warm handoffs to culturally responsive health, mental health, housing, and supportive service providers.
  • Coordinate with HCAO Be At School, schools, community partners, and service providers.
  • Support workforce readiness through career exploration, training referrals, job preparation, and connections to Workforce Strategies Coordinators.
  • Support participant access to employment pathways, credentialed training, and economic stability opportunities.
  • Maintain accurate participant files, case notes, Salesforce documentation, attendance, outcomes, and reporting requirements.
  • Assist with program evaluation, surveys, grant reporting, and continuous improvement efforts.
  • Participate in meetings and perform other duties as assigned.
