The Care Manager also addresses various patient needs including healthcare, finances, housing, and family dynamics, utilizing community resources effectively. They initiate referrals, document interventions, and perform reviews according to department standards. Collaboration with managers, physicians, and treatment teams is essential for understanding standards of care and determining appropriate site of care. The role requires staying knowledgeable about healthcare regulations, reimbursement, and community resources, and completing UM screenings for appropriate patient status. Developing and maintaining relationships with community agencies and serving as an educator to staff are also key responsibilities. The position requires the ability to provide age-appropriate care, demonstrating knowledge of growth and development across the lifespan.
