About The Position

Grafton Integrated Health Network is seeking an Early Intervention Speech Language Pathologist - PRN to help individuals with complex needs live meaningful, fulfilling lives. This role involves shaping a child's life during critical developmental stages by delivering person-centered, therapeutic interventions that support daily living skills, independence, and overall well-being. The position focuses on collaborating with individuals, families, and interdisciplinary teams to develop and implement individualized treatment plans. Grafton aims to remove administrative burdens, providing a laptop and cell phone to allow practitioners to focus on families. The role offers professional development opportunities to support growth.

Requirements

  • Requires a master's degree in speech language therapy.
  • Requires licensure issued by the Virginia Department of Health Professions.
  • Strong background in early childhood development required.
  • Requires ability to obtain Early Intervention Professional Certificate issued by the Part C office within two weeks of start date.
  • Valid driver’s license.

Nice To Haves

  • Prefer 1 year of clinical experience providing speech therapy; or any equivalent combination of training and experience which provides the required knowledge, skills and abilities.
  • Preferred - Existing Early Intervention Professionals Certificate issued by the Part C office.

Responsibilities

  • Provides developmental speech and language services in natural settings.
  • Provides services in accordance to the intensity, frequency and duration written on the Individual Family Service Plan (IFSP).
  • Completes documentation of services, in GIHN’s electronic client record, in accordance with Grafton, Medicaid, and Part C regulations.
  • Completes documentation of initial and on-going assessment in GIHN’s electronic client record in accordance with Grafton, Medicaid, and Part C regulation.
  • Provides all services using the Primary Provider Evidence Based Best Practice.
  • Initiates all services within 30 days of signature of the IFSP, unless there is clear and well documented evidence why that could not occur.
  • Keep all family appointments unless notified otherwise by the family.
  • Identifies and obtains necessary supplies and equipment.
  • Maintains a regular schedule of visits, to the best of their ability given family needs.
  • Participates in monthly staff meetings.
  • Provides all services in natural settings which include family homes, motels, daycare centers, parks etc.
  • May provide clinical supervision to non-certified speech-language pathologists.
  • Attends conferences and workshops to keep abreast of professional trends, developments and techniques.
  • Operates and programs a variety of augmentative communication devices.
  • Performs other duties as assigned.
