About The Position

Provides Early Intervention Services (EIS) to persons with HIV/AIDS with the purpose of establishing and/or re-establishing access to medical treatment. Assists People Living With HIV/Aids (PLWHA) in identifying and addressing barriers to the initiation of, participation in and adherence to on-going HIV outpatient/ambulatory medical care. Works collaboratively with outpatient case managers to identify and remove barriers for those who are not adequately accessing care.

Requirements

High School Diploma Required
Minimum 1 year experience in medical care, outreach, community health education, or human services field Required

Responsibilities

Initiates early intervention services within established policies, procedures and time frames to provide services appropriate to patient population.
Identifies people who are HIV positive who are not in care.
Locates patients who have fallen out of medical care, e.g., using Social Security Death Index, phoning patients’ emergency contacts while maintaining patients’ confidentiality, visiting the Food Pantry, etc.
Engages the client and forms a relationship of trust and understanding.
Links the client to medical care.
Makes home, hospital and non-traditional setting visits.
Assesses the clients’ social situation (i.e., health status, substance abuse history, mental health, educational needs, social support, etc.); works to remove barriers as it relates to barriers, health status (including mental health and substance abuse), and employment/educational conditions.
Follows-up with client’s progress.
Documents client interactions and services in CAREWare.
Communicates information gathered with appropriate personnel as required.
Works collaboratively with outpatient case managers and agencies, in a customer responsive and courteous manner, in compliance with MEDCARE Standards and promoting patient/customer satisfaction to identify and remove barriers for those who are not adequately accessing care.
Establishes close working relationships with agencies and services that are most likely to be accessed by persons in need of EIS services.
Performs service delivery, community/street outreach and monitoring.
Prepares and maintains required documentation, reports, records and files for operational, administrative and compliance purposes within established timeframes.
Utilizes computer to enter and retrieve client information; generate statistics, computations, tables or charts.
Provides clear, concise, timely written documentation and updates information and computer data as required.
Maintains confidentiality of all information in accordance with hospital, HIPAA, compliance and other regulatory requirements.
Attends continuing education, staff development, and mandatory programs.
