About The Position

Step into a meaningful role within our Hospice Care Team as a DME Coordinator dedicated to supporting patients and families during some of life’s most vulnerable moments. You’ll serve as a trusted resource for nurses, physicians, caregivers, and loved ones, helping maintain comfort, dignity, and continuity of care.

Requirements

  • High School Diploma or GED Degree or GED (Required)
  • 1 - 2 years experience Customer Service (Required)
  • 1 - 2 years experience Clerical (Required)
  • Computer aptitude
  • EPIC
  • Knowledge of durable medical equipment
  • Basic knowledge of Durable Medical Equipment

Responsibilities

  • In collaboration with the hospice interdisciplinary teams, obtains durable medical equipment in a timely and cost-effective manner, under the close supervision of the manager and/or assistant nurse manager.
  • Gathers information to successfully begin, support and/or complete DME ordering process, using established workflows & algorithms.
  • Accurately enters all appropriate information into portal & EMR in a timely manner.
  • Responds to calls promptly & courteously, taking appropriate action to resolve situations for positive outcomes.
  • Requests pick up of durable medical equipment after the patient discharge.
  • Facilitate recovery of lost DME.
  • Proactively problem solves, anticipates needs, and initiates solutions for both internal & external customers that support the successful completion of orders.
  • Works collaboratively with the interdisciplinary team & actively participates in team functions & meetings.
  • Adjusts to changes in workload & schedules based on changing departmental & organizational priorities.
  • Participates in internal & external education, training, in-services & other activities to promote personal & professional growth.
  • Adheres to established DME algorithms/workflows; identifies and works collaboratively with hospice team regarding changes/adjustments in workflows.
  • Excellence in interactions/communications with internal and external customers.
  • Requests/coordinates pickup of DME after patient discharge.
  • Responsible for oversight and facilitation process for recovery of lost DME.
  • Works collaboratively with the hospice team to process DME requests.
  • Accurately enters all appropriate information into electronic medical record and DME portals in a timely manner.
