About The Position

We are seeking a compassionate, skilled, and dual-certified Family and Psychiatric Mental Health Nurse Practitioner (FNP/PMHNP) to join our growing telehealth team. This role focuses on providing comprehensive, integrated care to patients with Substance Use Disorders (SUDs) and co-occurring mental and physical health conditions through secure virtual platforms. The ideal candidate is dedicated to evidence-based, patient-centered care and thrives in a collaborative environment that prioritizes access, empathy, and recovery support.

Requirements

Current certification as both FNP and PMHNP (dual-certified required).
Active, unrestricted NP license in the state(s) of practice.
DEA license with authorization to prescribe controlled substances (including buprenorphine waiver preferred or willingness to obtain).
Minimum 2 years of clinical experience as an NP, preferably in addiction medicine, psychiatry, or integrated behavioral health.
Strong knowledge of MAT protocols (Suboxone, Vivitrol, etc.) and evidence-based addiction treatment.
Excellent communication skills and comfort using telehealth platforms.
Ability to work independently while collaborating with a remote multidisciplinary team.

Nice To Haves

Experience working with underserved populations or community health settings.
Familiarity with trauma-informed care and motivational interviewing.
Knowledge of co-occurring chronic medical conditions common in SUD populations.

Responsibilities

Conduct comprehensive psychiatric and medical evaluations via telehealth for patients with SUD and co-occurring disorders.
Develop and manage individualized treatment plans that may include medication-assisted treatment (MAT), mental health support, and primary care interventions.
Prescribe and monitor psychotropic and primary care medications in accordance with state and federal regulations.
Provide ongoing follow-up visits to assess progress, adjust medications, and offer counseling and education.
Collaborate with an interdisciplinary team including therapists, case managers, and addiction specialists to ensure holistic, integrated care.
Maintain accurate and timely documentation within the electronic health record (EHR).
Participate in case reviews, team meetings, and continuing education to maintain clinical excellence.
Promote harm reduction, wellness, and recovery-based approaches in all patient interactions.
