About The Position

The DSS Care Coordinator is a Full Time, Non-Exempt position within the DSS Care Coordination program. The primary goal is to reduce and/or resolve the number of persons presenting to the Department of Social Services (DSS) as homeless who have been screened and identified as having Opioid Use or Polysubstance Abuse Disorder. This role involves meeting documentation requirements, maintaining current Care Plans, and ensuring compliance with applicable regulations. The coordinator provides assistance in crisis intervention and stabilization, and maintains regular contact with clients in their natural environment to monitor their status and well-being. A key responsibility is assisting clients in transitioning from inpatient or residential settings to community-based settings through service linkages and monitoring treatment progress, as well as assessing needs and personal goals by evaluating and interviewing the individual and their natural supports.

Requirements

  • Bachelor’s Degree in a Human Services field OR Associate’s Degree
  • Two (2) years’ experience working in direct care human services position
  • Functional knowledge of community resources
  • Education and Interest in training in Substance Abuse to increase skills and knowledge base
  • Openness to learning new skills and technology as necessary; must participate in training as required
  • Knowledge of computers and web-based programs
  • Valid New York State Driver’s License that meets Agency standards
  • Reliable Transportation
  • Strong organizational and analytical skills
  • Ability to think creatively and be results oriented.
  • Ability and willingness to abide by agency policies and procedures.
  • Ability to deal with multiple tasks efficiently (concurrent documentation during visits/verbal contact and collaborating with multiple providers)
  • Ability to represent self and agency in a professional manner.
  • Ability to engage and build a productive rapport with the people we serve and with outside providers/agencies.

Responsibilities

  • Meet Cattaraugus County and Intandem’s documentation requirements.
  • Maintain current Care Plan(s).
  • Ensure familiarity and compliance with applicable regulations.
  • Provide coordination and assistance in crisis intervention and stabilization.
  • Maintain knowledge of status and well-being of people we serve by maintaining verbal/face-to-face/electronic/written contact with the people we serve within their natural environment; at or above the frequency required by regulations.
  • Assist clients in transitioning from inpatient or residential to community-based settings via service linkages and monitoring the progress of the treatment and support services.
  • Assess needs and personal goals by evaluating and interviewing the referred individual/natural supports.
