About The Position

The person in this position will assist the Residential Services Manager in administrative functions and provide direct care support to residents. Ensure compliance with all regulations established by all relevant regulatory bodies and policies established by the CCARC Board of Directors. The person in this position will be current regarding trends and information as related to providing services to individuals with disabilities, enabling their highest quality of life in terms of their physical, emotional and mental health.

Requirements

  • High School Diploma/GED and three years experience working with individuals with developmental disabilities
  • Must have valid CT Driver's License, be insurable with Agency carrier. Individual Home Support employees are required to have a vehicle for use and maintain adequate insurance.
  • Must complete the Agency’s Employee Training Certification and training, including but not limited to: CPR, First Aid, PMT (Psychological/Physical Management Training) and DDS Medication Administration
  • Ability to follow oral and written instructions

Nice To Haves

  • Experience with Microsoft products, use of email and Therap preferred

Responsibilities

  • Assist the Manager in training staff on all individual programs and goals.
  • Review all individual program data to ensure accuracy and completeness.
  • Arrange coverage at the direction of the manager for planned staff absences or open positions. Assist as needed with on-call, ensuring that substitute and part-time staff are asked first to cover open shifts.
  • Assist the Manager with the monthly staffing schedule; obtain final approval from the Manager before posting.
  • Review and ensure accuracy of Kardex, doctor’s orders, and all required follow-up regarding individual medical concerns. Keep the Manager, Nurse, Day Services programs, and appropriate other parties apprised of any changes.
  • Serve as a mentor/role model to all direct support professionals and reinforce best practices.
  • Provides and receives effective, respectful, appropriate, and courteous verbal and written communications; to and from specific teams related to the care of participants, including daily observations and reports of any physical or behavioral changes while adhering to all documentation and record-keeping requirements.
  • Provides and receives effective, respectful, appropriate, and courteous verbal and written communications; With family members, guardians, DDS and external healthcare providers, including coordinating health and hygiene-related appointments.
  • Provides essential physical and emotional care to participants, including: Assisting with personal care, hygiene, and treatments, including administration of medications, recording vital signs as needed, and placing supply and medication orders.
  • Provides essential physical and emotional care to participants, including: Providing education to participants in personal hygiene, grooming, and behavioral conduct while acting as a role model to enhance participants’ self-esteem.
  • Provides essential physical and emotional care to participants, including: Coordinating and leading social and recreational activities as well as shopping for food and clothing, including the safe transportation of participants.
  • Provides essential physical and emotional care to participants, including: Coordinating General housekeeping as well as collecting and changing laundry.
  • Provides essential physical and emotional care to participants, including: Nutritional support: Monitoring residents at mealtime according to food consistency guidelines for each individual.
  • Demonstrates knowledge of emergency procedures and responds appropriately to emergencies.
  • Responsibly manages and records the use of household petty cash, food, and house money, and each individual’s money according to the policies established by the CCARC, Inc. Board of Directors.
  • Collaborating to resolve conflicts, obstacles, and challenges, seeking guidance from the manager and team members, as needed.
  • Demonstrates adherence to universal precautions, OSHA standards, and infection control policies and procedures at all times, as well as department, home and shift procedures.
  • Maintain good daily attendance (days of absence, illness, tardiness).
  • Attends and participates in mandatory in-services, staff meetings and conferences as required.
