Requirements

• One year as a Drug and Alcohol Case Management Specialist Trainee
• A Bachelor's Degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing or a related field; and one year direct service experience performing counseling or case management work in alcohol or other drug treatment setting
• A bachelor's degree which includes or is supplemented by successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice or other related social sciences; and one year direct service experience performing counseling or case management work in alcohol or other drug treatment setting
• Any equivalent combination of experience and training
• Meet the PA residency requirement
• Be able to perform essential job functions
• Pass a background investigation
• Pass a drug screening
• Satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance

Responsibilities

• Complete drug and alcohol screening and level of care assessments for all referred consumers
• Develop service plans and link consumers with community resources to meet determinants of health, and providers to implement the recommended level of care
• Track treatment progress and conduct required follow-up as desired by consumer, and document in required health record systems
• Support multiple specialty and problem-solving court programs
• Liaison with providers to ensure coordinated services
