About The Position

Elderhaus at the Lake, an Adult Day Health Center, provides compassionate, comprehensive care to help older adults remain independent and engaged in their community. This position is based in a welcoming Adult Day Health Center where the Personal Care Attendant (PCA) will build relationships, participate in activities, and create meaningful daily experiences for participants, while also supporting their overall health and independence. The role involves hands-on care and meaningful engagement throughout the day, including assisting with Activities of Daily Living (ADLs), leading activities, monitoring participant status, and supporting meals and environmental safety. Additionally, the role includes occasional transportation support, such as assisting with loading/unloading participants, ensuring safe transitions, and serving as an escort on transportation routes. The PCA will collaborate closely with nurses, coordinators, and the interdisciplinary care team, follow care plans, maintain documentation, and promote participant dignity, privacy, and independence.

Requirements

  • High School Diploma or equivalent required.
  • At least 1 year of experience working with elderly or frail adults required.
  • Valid driver’s license, insurance, and reliable transportation required.
  • Compassionate, patient, and service-oriented
  • Strong communication and teamwork abilities
  • Comfortable working in a fast-paced adult day center environment
  • Ability to support safe participant transport when needed

Nice To Haves

  • CPR certification preferred.

Responsibilities

  • Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
  • Lead and participate in group and one-on-one activities that promote socialization and well-being.
  • Monitor participants’ physical and emotional status and report changes to the care team.
  • Support meals, laundry, and maintaining a clean, safe environment.
  • Assist with loading/unloading participants and ensuring safe transitions to and from vehicles.
  • Serve as an escort on transportation routes as needed to support participant safety.
  • Provide hands-on assistance to participants during transport when appropriate.
  • Work closely with nurses, coordinators, and the interdisciplinary care team.
  • Follow individualized care plans and maintain accurate documentation.
  • Promote participant dignity, privacy, and independence.
