Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community. As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.

Requirements

  • Valid CA Class A, B, or C commercial driver’s license (CDL) with passenger endorsement required
  • Clean driving record (free from major infractions including, but not limited to, DUI, negligent, reckless, or excessive minor infractions and convictions) required
  • Current Basic Life Support (BLS) certification is required upon hire and must be maintained as a condition of employment. These courses must follow AHA guidelines, but may be completed through approved online providers such as ProMed or other equivalent programs that meet recognized BLS standards
  • Demonstrated ability to safely operate a wheelchair accessible van or vehicle.
  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills.
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
  • Ability to successfully manage multiple tasks simultaneously
  • Excellent planning and organizational ability
  • Ability to work as part of a team as well as independently
  • Ability to work with highly confidential information in a professional and ethical manner

Nice To Haves

  • High school diploma/GED preferred
  • Two years medical, ADA, pupil, and/or elderly transportation experience preferred
  • Successful completion of defensive driving and passenger assistance training courses within three months of hire

Responsibilities

  • Operate a 12-passenger bus safely in accordance with traffic laws and Neighborhood’s policies.
  • Transport participants to and from the day center, medical facilities, and other scheduled outings.
  • Assist participants in boarding and exiting the vehicle safely, including those using mobility aids such as walkers or wheelchairs.
  • Conduct pre-trip and post-trip vehicle inspections and maintain accurate records.
  • Maintain cleanliness and basic upkeep of the vehicle.
  • Report any incidents, accidents, or participant concerns to appropriate staff promptly.
  • Communicate effectively and courteously with participants, caregivers, and team members.
  • Maintain confidentiality and respect participants’ rights and privacy.
  • Participate in training and meetings as required.
  • Provides transportation for passengers in a timely and competent manner which ensures patient and participant satisfaction and retention; escorts patients to and from buildings when necessary
  • Operates equipment safely and in compliance with vehicle licensing regulations
  • Reports participant or caregiver requests for changes in drop-off or pick-up locations
  • Follows established schedules and makes necessary adjustments to meet unanticipated changes; determines the safest and most efficient pick-up and delivery routes while considering the needs of participants and requirements of clinic and therapies and maintains narrative of route
  • Carries or assists one-person transfers of participants to and from vehicles and inside vehicles
  • Inspects and ensures vehicles are maintained to a clean and safe condition and gas tank filled each night; notifies appropriate supervisor of maintenance or repair needs
  • Maintains standard records, (e.g., logs of passengers and miles driven, service records, gasoline purchases)
  • Promptly reports to appropriate staff regarding participant-related concerns noted in home or on the bus; writes progress notes for documentation
  • Delivers medications and other supplies to participants
  • Follows all safety guidelines and standards; attends safety in-services and offers guidance or assistance to staff working in an unsafe manner
  • Reports accidents and incidents to supervisor immediately
  • Follows proper infection control procedures and maintain supply of emergency clean up kits
